It wasn't the coming back itself, it was his wife's betrayal, specificly when she through the antiserum into the mix... It caused everything to discombabulate (i upendo that word) and he never came fully back. So in order to stay alive he had to absorb life energy, because he was burning his own. The Doctor explains that right before Donna's Grandad shows up. (I dont remember his name at this very moment... Pregnancy brain... Sorry)
